One of our Case Managers will meet with you and your family member(s) to better understand your care needs and identify how Companion Care can help.
Plan of Care Agreement
We identify the client’s needs in the Plan of Care so that each caregiver knows what is required to care for the client.
Personally Introduce Caregiver
Using the information we gather during our initial meeting, we will identify the caregiver that best matches your needs and schedule a time for you to meet. We like making the initial introduction to ensure that the client is comfortable and happy with the caregiver. This also gives family members the peace of mind that their loved one is being well cared for.
Ongoing Care Review
By working with our clients, we are able to identify and report when their needs change. We work closely with each member of our client’s care team (other health professionals, social workers, family members, etc.) to provide the best care.
